Reference is made to your recent letter in which you requested an evaluation of an unidentified flying object which you observed.

FTD Form 164 which is used for reporting such objects to the Air Force is attached. This form should be completed as fully as possible and returned to this office for evaluation.

Dear Information Officer,

I would like to make a request for a report on any and all unidentified flying objects, (U.F.O.), and flying saucers, as well as sightings of these concerned or connected with Project Blue Book. I would also like any other information that would be available on this subject.

Now I would like to report a sighting of a mysterious object. It happened one night in the mouth of June this year. I am in the band and we went to Sarasota for a parade. We were off the bus at about 7:00 and we were standing around waiting just off the highway a few miles out of town. I was loo
