Sir:

Please send me a copy of the standard Air Force
questionnaire regarding sightings of flying saucers. Also
send me the more detailed questionnaires, Form A and
Form B, comprising 10 pages of sightings. I am requesting
these questionnaires because of the fact that, on October
31, 1938, that I saw a new flying saucer. Below you
will find some of the information on that sighting.

Remarks: I saw the saucer 3 miles from Tilden East (1 mile from Meno Road). The object was first sighted off in the distance of a minute. Never could possibly
in fact, very quickly, the object flew towards me. I was standing on a road at the time. The object came directly up to within 300 feet of me at an altitude of 1 mile. Then suddenly, it started to attempt a landing, which it never did accomplish

flying saucer as it appears to have to run on
Coffin 31, 1938. It will give you an idea of what
the saucer looks like.
