This is to acknowledge your telephone contact with this Center on the morning of 4 April 1958 regarding a sighting of an unidentified flying object, and to confirm a conversation between you and Capt. Gregory regarding an arrangement for an interview.

1. Received attached "buck slip" at 1045 hours today re what is believed to be the "Columbus UFO" which the press has contacted both the AF and the Base.

2. In conversations with Mr. Hieatt, Public Relations Specialist for ATIC, who contacted this office early this morning, it was pointed out no official notice or report of the incident was made to ATIC.

3. The press was notified accordingly.

Subject: Follow-up on UFO Questionnaire

1. Mrs. [redacted] was contacted by phone, this date, regarding the status and disposition of the UFO questionnaire sent to her on 3 April 1938.

2. It was reiterated to Mrs. [redacted] that:
a. She volunteered the original information late on the night of 3 April 1938 to the Duty Officer.
b. She agreed to fill out a questionnaire, as her original report was lacking since vital dates; and that there were certain inconsistencies.
