Dear Sir,

I need a book of 400's sometime ago. Yesterday evening I saw an object which I believe was an U.F.O. If you wish a written statement besides the questions, I will readily compare the names of the person who also sees the U.F.O. was my sister.

Troops this questionnaire will help you in your research. You have my permission to use my name in connection with any statements, conclusions, etc.

Please excuse the writing, Sir, but I am writing the enclosed. I should be in bed, the time being 2 minutes & 0 seconds after midnight Sunday, 17th January 1965.

Yours faithfully
